Block Schedule
In response to feedback from past residents, the Internal Medicine Residency Program began a new block schedule (x plus y model) in June 2022. The new schedule has been very successful, with improved opportunities for learning during both inpatient and dedicated ambulatory weeks.
Residents rotate through an ambulatory week in a rhythm of three plus one. For blocks of 3 weeks, the residents rotate through non-ambulatory week rotations, including inpatient wards, ICU, night shifts, electives and vacations, then rotate through a focused ambulatory medicine training week. During this ambulatory week, they experience a mix of continuity clinics, walk-in clinic, mentored in-basket management sessions, community health rotations and attending-led didactic sessions.

Whether a resident is certain about pursuing a hospital-based career, a primary care practice, a subspecialty fellowship, or are still undecided, the new block schedule experience provides the opportunity to gain valuable skills in the ambulatory setting. Every ambulatory week also involves one half day of dedicated didactics, led by Dr. Perrin, co-director of Ambulatory Care Education.
The block schedule provides a structure for each resident’s yearly schedule. Every resident rotates through each of their ambulatory weeks with the same cohort of five other residents throughout the year. Each cohort is comprised of a mix of senior residents (PGY2,3) and interns (PGY1), and during the ambulatory weeks, the residents enjoy no evening or inpatient work as well as a full weekend off at the end of the week, before beginning the next rotation.
The general breakdown of the annual schedule for 2024-25 is outlined below.

PGY-1
- Inpatient Medicine 20 weeks
- Night Cross Cover Inpatient Medicine 4 weeks
- ICU 4 weeks
- Night ICU Coverage 4 weeks (note, 1 week = 3-4 nights, with remaining days off)
- Electives 12 weeks
- Ambulatory 8-9 Weeks
- Vacation 3 weeks
PGY-2 & PGY-3
- Inpatient Medicine 16 weeks
- ICU 3 weeks
- Night Float 2-3 weeks
- Swing Shift 2-3 weeks
- Electives 10 weeks
- Intro to Quality Rotation (PGY-2 only) 2 weeks
- Ambulatory 8-9 Weeks
- Vacation 4 weeks
