GOAL

To recruit and retain talented and high performing employees by offering a comprehensive and competitive total compensation package, including various insurance and retirement benefits.

POLICY

Cottage Health System (CHS) provides eligible employees, in-house travelers, and resident physicians with certain insurance and retirement benefits as part of their overall total compensation package. This policy is for general guidance and is not intended to override the actual Plan Documents or Insurance Contract. Should there be a discrepancy between this policy and the Plan Document or Insurance Contract, the Plan Document or Insurance Contract will be used for determination.

PROCEDURE

CHS offers the following insurance benefits, depending upon Employee, Resident Physician, or In-House Traveler classification:

EMPLOYEE

Health (Medical, Dental, Flexible Spending Accounts)

  • Eligibility (Part-Time Benefits): Regularly scheduled to work between 40-72 hours per pay period and classified in a part-time benefit-eligible status (excluding Standby hours).
  • Eligibility (Full-Time Benefits): Regularly scheduled to work 72 hours or more per pay period and classified in a full-time benefit-eligible status (excluding Standby hours).
  • Effective Date: First of the month following 30-days of continuous employment from date of hire or date of hire for Department Directors and Executives. If current employee transferring to an eligible status, first of the month following transfer date.
  • Coverage: For details regarding insurance coverage, refer to the Summary Plan Description distributed at time of enrollment or available from Benefits Administration.
  • Coverage End Date: Last day of pay period based on termination date or date transfer to an ineligible status.

Life and Long-Term Disability

  • Eligibility: Regularly scheduled to work 40 or more hours per pay period and classified in a part- or full-time benefit-eligible status (excluding Standby hours).
  • Effective Date: First of the month following 30-days of continuous employment from date of hire or date of hire for Department Directors and Executives. If current employee transferring to an eligible status, first of the month following transfer date.
  • Coverage: For details regarding insurance coverage, refer to the Summary Plan Description distributed at time of enrollment or available from Benefits Administration.
  • Coverage End Date: Last day of pay period based on termination date or date transfer to an ineligible status.
  • Conversion: Upon termination, employees covered in a CHS Life and/or LTD plan may be eligible for a private policy based on the terms of the conversion policy. Additional information may be obtained from Human Resources within 30-days of termination.

RESIDENT PHYSICIAN

Health (Medical and Dental)

  • Eligibility: Resident physicians receive the High Medical and Dental plans.
  • Effective Date: Date of Hire
  • Coverage: For details regarding insurance coverage, refer to the Summary Plan Description distributed at time of enrollment or available from Benefits Administration.
  • Coverage End Date: Date contract ends.

Life and Long-Term Disability

  • Effective Date: Date of Hire
  • Coverage: For details regarding insurance coverage, refer to the Summary Plan Description distributed at time of enrollment or available from Benefits Administration.
  • Coverage End Date: Date contract ends.

Retirement:

  • None

IN-HOUSE TRAVELER

Health (Medical and Dental)

  • Eligibility: In-House Travelers receive the Basic Medical and Dental plans. No dependent coverage is available.
  • Effective Date: First of the month following 30-days of continuous employment from date of hire.
  • Coverage: For details regarding insurance coverage, refer to the Summary Plan Description distributed at time of enrollment or available from Benefits Administration.

Life Insurance

  • Eligibility: Regularly scheduled to work 40 or more hours per pay period and classified in a part- or full-time benefit-eligible status (excluding Standby hours).
  • Effective Date: First of the month following 30-days of continuous employment.
  • Coverage: Eligible for basic life insurance of $10,000. For details regarding insurance coverage, refer to the Summary Plan Description distributed at time of enrollment or available from Benefits Administration.
  • Coverage End Date: Last day of pay period based on termination date or date transfer to an ineligible status.
  • Conversion: Upon termination, employees covered in a CHS Life plan may be eligible for a private policy based on the terms of the conversion policy. Additional information may be obtained from Human Resources within 30-days of termination.
