CottageOne Link is Cottage Health's free and convenient web-based portal for you to securely access electronic medical records for your patients at Cottage Health.

The CottageOne Link application can be securely accessed 24/7 from your internet browser – no software installation is required – and is HIPAA compliant for fast and easy access to clinical and billing data including patient chart, lab/med/procedure order details, and patient demographic and insurance information.

Organization Profile

Organization Profile and Use of Online Services Agreement – to be completed and electronically signed by an owning partner of your organization.

End User Request – online form for Site Administrators ONLY

End User Request Form and Universal Confidentiality of Information Acknowledgement – to be completed and signed by each individual user requesting access, with proper signatures.

NOTE: There must be an Organizational Profile on record for the organization you represent.

Once your enrollment is processed, you will be contacted by our Identity Access Management team with instructions on how to receive your credentials.


Profile Definitions

Billing Staff
Responsible for billing for community provider services, with limited access to CottageOne Link functionality to review medical services and associated charges for billing purposes. User can access chart review, problem list, and history as well as coverage benefits and eligibility.

Clinical Support Staff
Access to clinical information to support their associated providers in patient care. User roles in this category include RN and MA.

Front Desk
Scheduler or front desk staff member at a community practice. Access to functionality in CottageOne Link is limited to primarily patient demographics.

Provider
User role designated primarily for MDs, PAs, and NPs in community clinics, with broad access to CottageOne Link's functionality including viewing patient data.

ROI Requester
Third party user requesting information from Health Information Management (HIM) – for example, patients who need immunization records, as well as attorneys and courts that review care received at the organization.

Site Administrator
The Site Administrator has the responsibility and security to manage the CottageOne Link accounts of other users at their organization. This includes editing user demographics, resetting passwords, requesting new user accounts, and deactivating user accounts.
