Vaccine Records
You will receive a paper vaccine record card when you are vaccinated.
If you were vaccinated in California, you may get a digital COVID-19 Vaccine Record from the State of California.
If you received vaccine from Cottage Health, you can access your medical record through MyChart or by contacting the Health Information Management office.
